What is a Personalized Medication List?

  • A personalized medication list is a record of all prescription medicines, over-the-counter (OTC) medicines, vitamins, supplements, and herbals that you use. This includes medications that you take on a daily basis and anything you use as needed.
  • A personalized medication list should also include the medication strength, dose, how you take the medicine (by mouth/ into the eye, etc.), how often you take the medicine, and at what time you take the medicine.
    • Example: Lipitor 20mg, Take 1 tablet by mouth everyday at bedtime.

Why is a medication list important?

  • Helps decrease medication errors and improve patient safety
  • Helps your doctor decide on the best medications for you
  • Helps prevent drug-drug interactions, drug-food interactions, and duplicate therapies

  • Share your medication list with every health care provider you see.
    • This includes all doctors you see, your pharmacist, your dentist and any hospital you go to.

When should I update my medication list?

  • Your medication list should be updated each time you add, stop, or change a medication. For example, this could happen after your doctor’s visit or after you are discharged from the hospital.
  • To make updating your list easy, you can use a form to make your own list, or you can ask your pharmacist to help you make a list.
